Seasonal shifts and prolongation of the pollen season caused by climate change have\u00a0<a href=\"https:\/\/www.lungenaerzte-im-netz.de\/krankheiten\/asthma-bronchiale\/haeufigkeit\/\" target=\"_blank\" rel=\"noopener\">Allergic respiratory diseases have increased sharply since the 1980s.<\/a>which affects the frequency of prescribing DAe and PI.<\/p><p>Daily medication against allergic asthma is prescribed in two different forms of application, among others:<\/p><ol><li><strong>Metered dose inhalers (DAe)<\/strong> are applied to the mouth. By applying pressure to the trigger, a propellant with active ingredient is delivered into the respiratory tract with simultaneous inhalation.<\/li><li><strong>Powder inhalers (PI)<\/strong> are enclosed by the mouth. Inhalation takes place by suction during rapid inhalation.<\/li><\/ol><h3>Propellants of metered dose inhalers are more harmful to the climate than powder inhalers<\/h3><p>In addition to the greenhouse gas carbon dioxide, whose share has risen sharply since industrialisation, other greenhouse gases such as\u00a0<a href=\"https:\/\/agderma.de\/en\/topics\/plastic-causes-climate-change\/\" target=\"_blank\" rel=\"noopener\">Methane<\/a>nitrous oxide and man-made fluorinated greenhouse gases (F-gases) are released into the atmosphere. F-gases do not affect the ozone layer, unlike CFCs, whose production has been banned worldwide since 2020.<\/p><p>F-gases are not only found in products such as freezers, air conditioners, fire extinguishers and insulation materials, but also in asthma sprays. The\u00a0<a href=\"http:\/\/www.duh.de\/uploads\/tx_duhdownloads\/F-Gase_Hintergrundpapier.pdf\" target=\"_blank\" rel=\"noopener\">Global warming potential of these gases is about 100 to 23,000 times higher<\/a>\u00a0as that of carbon dioxide. The\u00a0<a href=\"https:\/\/www.umweltbundesamt.de\/daten\/klima\/atmosphaerische-treibhausgas-konzentrationen#beitrag-langlebiger-treibhausgase-zum-treibhauseffekt\" target=\"_blank\" rel=\"noopener\">Dwell time in the atmosphere<\/a>\u00a0is in some cases between a few decades and several hundred years. Worldwide, they contribute around\u00a0<a href=\"https:\/\/bmjopen.bmj.com\/content\/9\/10\/e028763\" target=\"_blank\" rel=\"noopener\">two per cent to the greenhouse effect<\/a>\u00a0and thus about as much as international air traffic. Previous research has shown, for example, that DAe prescribed in England contribute 3.9 % to the carbon footprint of the UK National Health Service. In Germany, F-gas emissions already accounted for\u00a0<a href=\"https:\/\/www.umweltbundesamt.de\/daten\/klima\/treibhausgas-emissionen-in-deutschland\/emissionen-fluorierter-treibhausgase-f-gase\" target=\"_blank\" rel=\"noopener\">Federal Environment Agency<\/a>\u00a0accounts for about 1.6 per cent of greenhouse gas emissions.<\/p><p><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/33124196\/\" target=\"_blank\" rel=\"noopener\">The F-gases used in asthma sprays<\/a>mostly tetrafluoroethane HFA 134a and heptafluoropropane HFA 227ea, are powerful greenhouse gases and up to 1,430 and 3,220 times more potent than carbon dioxide respectively.<\/p><h3>Comparison of metered dose inhalers (DAe) - powder inhalers (PI)<\/h3><p>If we now compare the carbon dioxide balance of DAe with PI, that of\u00a0<a href=\"https:\/\/thorax.bmj.com\/content\/75\/1\/82\" target=\"_blank\" rel=\"noopener\">PI up to 20 times better<\/a>\u00a0as\u00a0<a href=\"https:\/\/www.aerzteblatt.de\/n107077\/\" target=\"_blank\" rel=\"noopener\">which DAe<\/a>.<\/p><p>Most of the active substances and combinations of active substances that play a role in the therapy of asthma and COPD are available both as DAe and PI.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-5d58bc7 elementor-widget elementor-widget-image\" data-id=\"5d58bc7\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"image.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t\t\t\t<figure class=\"wp-caption\">\n\t\t\t\t\t\t\t\t\t\t<img fetchpriority=\"high\" decoding=\"async\" width=\"625\" height=\"120\" src=\"https:\/\/agderma.de\/wp-content\/uploads\/2023\/01\/Tabelle-Dosieraerosole.webp\" class=\"attachment-large size-large wp-image-1195\" alt=\"\" srcset=\"https:\/\/agderma.de\/wp-content\/uploads\/2023\/01\/Tabelle-Dosieraerosole.webp 625w, https:\/\/agderma.de\/wp-content\/uploads\/2023\/01\/Tabelle-Dosieraerosole-300x58.webp 300w\" sizes=\"(max-width: 625px) 100vw, 625px\" \/>\t\t\t\t\t\t\t\t\t\t\t<figcaption class=\"widget-image-caption wp-caption-text\"><\/figcaption>\n\t\t\t\t\t\t\t\t\t\t<\/figure>\n\t\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-a5951ba elementor-widget elementor-widget-text-editor\" data-id=\"a5951ba\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>* In\u00a0<a href=\"https:\/\/www.cme-point.de\/Fortbildungen\/2589_CME_Inhalationssysteme.pdf\" target=\"_blank\" rel=\"noopener\">Studies<\/a>\u00a0it was shown that the effectiveness of the respective systems DAe versus PI does not depend on the type of drug, but on the correct application.<\/p><p>** cf. <a href=\"https:\/\/bmjopen.bmj.com\/content\/9\/10\/e028763\" target=\"_blank\" rel=\"noopener\">https:\/\/bmjopen.bmj.com\/content\/9\/10\/e028763<\/a><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-b668183 elementor-widget elementor-widget-text-editor\" data-id=\"b668183\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p><strong>Recommendation for the prescription of asthma medication:<\/strong><\/p><ul><li>Consider prescribing powder inhalers if a deep breath of 2-3 seconds or a longer breath of 4-5 seconds is possible, as these have a much better climate balance.<\/li><li>When using MDIs as an emergency medication or for elderly people, careful information should be provided not only on their use but also on their disposal.<\/li><li>Note the propellants in metered dose inhalers, as these differ significantly in terms of their carbon footprint.<\/li><li>Metered dose inhalers <a href=\"https:\/\/agderma.de\/en\/sustainable-practice-management\/sqm-templates-2\/\">dispose of correctly<\/a>as residual gases escape after the end of use and the inhalers are dangerous goods.<\/li><li>Disposal services for medicines depend on the municipality.
